Program Eligibility
In order to be eligible to recieve appropriate accommodations
or academic adjustments, each student must:
1. Self-disclose
his or her disability the Coordinator of the Office of Disability Services.
2. Provide documentation from a licensed
professional regarding the nature of his or her disability, including:
-diagnosis of the condition and how it substantially limits a major life activity;
-the condition's duration (is it permanent? short-term?);
-any accommodations the at the professional recommends.
It is important to remember that the accommodations recommended
by the professional may be beyond the obligation of the University (for example,
personal attendants, mental health counseling, etc.).
3. Sign registration forms designed
to assist in the development of a program of accommodations and to protect his
or her confidentiality.
